Go back to Service Policy. DepositsDeposit policies shall be applied without regard to race, color, creed, sex, age, national origin, or marital status. Deposits may be required of any Residential customer before service is supplied. These deposits or suitable guarantees may be based in whole or in part on information obtained in a report from a consumer reporting agency. Deposits approximately equal to twice the average monthly electric and water bills may be required of any General Power (Commercial) customer before service is supplied. If General Power customer requests, deposit requirements equal to or in excess of $200.00 may be met by customer purchasing a time deposit or comparable low risk, short-term investment which is acceptable to Mayfield Electric & Water Systems. Any cash deposit which is retained shall earn interest at the rate required by applicable K. R. S. standards and shall be paid when service is disconnected or paid annually upon written request of customer. Upon termination of service, any retained deposit and accrued interest shall be applied against unpaid bills of customer and if any balance remains after such application said balance shall be refunded to customer. DISCLOSURE OF USE OF INFORMATION OBTAINED IN A REPORT FROM A CONSUMER REPORTING AGENCY:MEWS’ credit decision may be based in whole or in part on information obtained in a report from the consumer reporting agency listed below. The reporting agency did not make the decision and is unable to provide you with the specific reasons for the decision. Because information from a consumer credit report was used in evaluating your application, you have the right under the Fair Credit Reporting Act to request a free copy of the credit report from the company listed below within sixty (60) days of your receipt of this notice. If, after receiving a copy of your credit report, you determine that the information is not accurate or complete, you have the right to request that the reporting agency listed below reinvestigate that information.

Guarantor's AgreementA guarantor's agreement can ensure payment of utility bills in the event an applicant for service can not pay a cash deposit. If the applicant for service has a co-signer/guarantor (Guarantor must be a city of Mayfield home-owner and current electric service customer of MEWS) who will agree to provide security for his account in lieu of paying a deposit, the guarantor may come into the MEWS office with the applicant for service to complete the Guarantor's Agreement form. Application must be made at our main office but you may print the Guarantor's Agreement, fill it out, and bring it to our main office. For More Information Contact: |
